PAR - Friends Entry Form

All it takes is 3 easy steps to add or update your child's photo to the Photo Gallery.

Provide the following information (if you don't have a digital image file saved on your computer then check the box requesting more information on where to send printed copies) and press Submit.

You will then be given the opportunity to upload up to 4 files at once or return to the website if you don't have a digital image.

Upon receipt, we'll add the information to the Photo Gallery and send you a confirming email so you can check the addition.
